Implant shapes vary between round and anatomical (teardrop-shaped). While some surgeons prefer anatomical implants for their supposedly more natural shape, Dr. Ronan often recommends round implants.

He points out that round implants naturally take on a teardrop shape when the patient is standing due to gravity and flatten out when lying down – mimicking the behavior of natural breasts.

Dr. Ronan’s surgical approach sets him apart from many of his peers. While the inframammary fold (under the breast) and periareolar (around the nipple) incisions are common, Dr. Ronan often prefers a less conventional method: the transaxillary approach, or armpit incision.

“The most common one in our practice is through the armpit,” he reveals. This technique offers several advantages:

  1. No visible scarring on the breast
  2. Reduced risk of sensation loss
  3. Preservation of chest muscle integrity

Dr. Ronan elaborates, “When we do it transax, we don’t have to cut your chest muscle, and so your chest muscle acts like a sling holding your implant up against gravity over time.”

This method not only maintains muscle function but may also contribute to better long-term results and a more natural appearance.

Implant Placement: Under the Muscle

When it comes to implant placement, Dr. Ronan typically favors submuscular positioning, where the implant is placed beneath the chest muscle. This approach can provide additional coverage, potentially reducing the risk of visible implant edges, especially in women with minimal existing breast tissue.

The submuscular placement also aligns well with Dr. Ronan’s preference for the transaxillary incision. By going under the muscle, the implant is well-supported and less likely to descend over time, a phenomenon known as “bottoming out.”

Recovery and Pain Management

One of the primary concerns for many women considering breast augmentation is post-operative discomfort. Dr. Ronan’s practice employs an innovative pain management technique to address this issue.

“We actually put a three-day numbing medicine around the implant and around your incision, and that really takes the edge off,” he explains.

This approach significantly reduces sharp pain in the immediate post-operative period. However, patients may still experience some muscle tightness, especially with submuscular placement. Dr. Ronan advises patients to manage this by consciously relaxing their shoulders and gently stretching the chest muscles.

Most patients can return to light activities within a week, with a full return to strenuous exercise typically occurring around 6-8 weeks post-surgery.

A common question among potential breast augmentation patients concerns the lifespan of implants. While the FDA recommends exchanging implants every 10 years, Dr. Ronan offers a more nuanced perspective:

“It lasts until you have a problem or need to make a change, and so that could be a very long time.”
