How do we work with your health plan?
We are a “fee for service” center, you are responsible to pay for services at the time of the visit and can submit for reimbursement to your insurance plan.
Use the superbill (receipt) from your visit and upload it to the app called “Reimbursify”. This app works for any “out of network” office visit with any insurance company and may reimburse you directly (generally about 50% of your consult fee and 40% of your procedure fee). Does not work for HMO, Kaiser or medicare plans.
Botox for Chronic Migraine
If you are a PPO or Medicare patient, we will work with your insurance company to get approval for Botox and the injection fee by submitting information that was gathered during your consultation visit. This may take 2-4 weeks depending on your health plan. Please make sure we have a copy of your insurance card so we can begin the authorization process.
Your health plan may cover the Botox but require that the Botox is sent to us from a specialty pharmacy. The specialty pharmacy will contact you directly via phone. It is important that you speak to them to ensure proper delivery of your Botox. You may need to provide a required out-of-pocket cost for the drug prior to being shipped. However, most plans cover the majority of the Botox cost.

Depending on your requirements of your insurance company, we will need to obtain approval for ongoing treatment at certain intervals. In order to assist with this, it is important you follow up 6-weeks after your injections for documentation of effectiveness and to keep a headache diary, so we can ensure treatment at proper intervals.
If you have Kaiser, HMO or other plans not covered; you have the option to pay cash for your Botox treatment at any time for the fee of $1600 (includes 155 units of Botox and injection fee)
Nerve Blocks for an Acute Treatment
If you have an acute headache not responding to your rescue medications, you can come in for a nerve block and trigger points. Please call to verify provider availability. The nerve blocks generally abort an acute headache and can keep you headache free for several days. We use a long acting lidocaine called Marcaine, there is no downtime and you can drive yourself to and from the appointment.
The cost of the nerve block with trigger points is $200, you can try submitting for reimbursement (see above) however, most insurance companies do not cover nerve blocks at this time.
Migraine Surgery refers to surgical decompression of one or several nerves in the head and neck which have been shown to trigger migraine symptoms in many migraine sufferers. Migraine surgery is a highly individualized procedure. Studies have shown that up to 95% of patients undergoing surgery, experience improvement or complete elimination of migraine headaches. The BPS migraine specialists here will work together to see if you may be a surgical candidate.
